Assumption of Risk
I have chosen to participate in the above activity or program (The Activity), and do so in full recognition and appreciation of the dangers and hazards inherent in The Activity. Participation in The Activity carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injury. The specific risks vary from one activity to another, but the risks range from 1) minor injuries such as scratches, bruises, and sprains, to 2) major injuries such as eye injury, joint or bone injuries, heart attacks, and concussions, to 3) catastrophic injuries such as paralysis and death.

Release of Liability & Indemnification
I do, for myself, my heirs, successors, assigns, and personal representative(s), hereby agree to release Saint Leo University, its Board of Trustees, officers, agents and employees from any and all liabilities, claims, demands or actions, whether in law or in equity and whether in tort, including negligence, or in contract, on account of any property or personal losses, damages and expenses, including reasonable attorney fees, or on account of personal injury, death, or emotional distress which may result, directly or indirectly, from my participation in The Activity, unless any such damage or injury is primarily the direct result of gross negligence or intentional misconduct of the University or any of its officers, employees or lawful agents and not caused in part by my own negligence. This Agreement and Release, in its entirety, shall run to and be binding upon me and my heirs, assigns or estate. It shall be governed by the State of Florida. Should any provision of this agreement be found to be unenforceable, all remaining provisions will remain in full force and effect. I HAVE CAREFULLY READ THIS AGREEMENT AND UNDERSTAND ITS TERMS AND CONDITIONS. I RECOGNIZE THAT I AM GIVING UP SUBSTANTIAL RIGHTS, INCLUDING THE RIGHT TO SUE, AND I REPRESENT THAT I AM SIGNING THIS AGREEMENT VOLUNTARILY, THAT I INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY. I UNDERSTAND THAT THIS AGREEMENT IS A LEGALLY BINDING CONTRACT AND I HEREBY AGREE TO ABIDE BY THE TERMS AND CONDITIONS AS OUTLINED HEREIN.
